We are seeking a Service Coordinator to join our team!
* Candidates must live in or near the greater Scranton city area and willing to travel in neighboring counties.
Service Coordination is the process of working with participants under a waiver program in order to identify, coordinate, and facilitate all necessary health / medical services based on their needs and goals.
You will provide administrative support to clients in either individual or group settings.
*Responsibilities:*
* Assess nature of clients' situations by interviewing and reviewing personal history
* Develop and execute individual treatment plans
* Determine appropriate milestones to gauge client progress
* Communicate client updates to all relevant parties
* Facilitate referrals to other healthcare professionals and programs
* Maintain accurate client documentation
* Meet important deadlines & requirements
* Travel throughout Pittsburgh & surrounding counties
\*\*Must have reliable transportation- The SC conducts 4 face to face visits per week.
\*\*\* Must have a valid driver license.
*Qualifications:*
*Bachelor’s degree in social work, psychology, human services or related field*
*OR*
*At least 3+ years of experience in social service or health care related setting*
